Caring for Transitioning Patients

However we go above and beyond our business requirements by working with stakeholders to improve the healthcare system.  A good example of this is our involvement with the National Transitions of Care Coalition (NTOCC) which is dedicated to improving transitions of care. Sanofi US felt so strongly about this issue that in 2006, it co-founded NTOCC with the Case Management Society of America.

Improper transitions of care can be caused by poor communication and coordination between health care professionals, patients and care givers that can lead to serious and even life-threatening situations. In addition, these inefficiencies waste resources and frustrate health care consumers.

Due to the nature of our business, Sanofi US is committed to ensuring that the healthcare environment allows patients to benefit from:

  • Products and services through coordinated care;
  • Appropriate hand over of patient care plans (including appropriate medications) from one provider/care setting to another during transitions and,
  • Proper follow up care that would prevent unnecessary re-hospitalization.

As NTOCC Board chair, Sanofi US provides leadership in executing ideas and strategies brought forth by a multi-organization advisory board.  The advisory board includes organizations representing healthcare professionals from various settings in the healthcare continuum. They share their expertise with NTOCC about the best ways to affect change and improve the care of transitioning patients.

At NTOCC’s inception, the coalition was comprised of 13 organizations that grew to 32 representing millions of consumers.  Today, NTOCC is a 501 (c)4 organization.  It accomplishes its mission through work groups that focus on the following areas:

  1. Developing tools and resources to improve transitions
  2. Promoting legislation and policies that provide for and encourage coordinated care during transitions
  3. Measuring the impact of and outcome of improved transitions
  4. Promoting health information technology that contributes to improved transitions.

Over the past 6 years, NTOCC has touched 30 million consumers and 200,000 health care professionals.   NTOCC has over 4500 subscribers using the website on a regular bases and over 149 countries access the resources to enhance transitions of care.
